ABC Daycare Admission

ADMISSION

All children are required to have completed enrollment forms on file in the Daycare Center prior to their first day of attendance in school. No child is considered enrolled until all forms are completed and all fees are paid in full. Forms include:

1. Child Enrollment Informaiton Form

• Identification and Emergency Information Form
• Child’s Pre-admission Health History-Parent’s Report
• Notification of Parents’ Rights Form
• Personal Rights Form
• Consent For Emergency Medical Treatment
• Descripline Policy
• Waiver of Claims for Transportation of Students
• Parent Pledge

2. Medical Information Signed by a Physician

• Physician’s Report
• Current Immunization Records
• TB Examination with Negative Results

3. Admission / Payment Agreement

Time 6:30 am to 6:00 pm
Child must be 2 years old by September 1st
Enrollment Fee           $175.00
Re-Enrollment Fee     $  50.00
Weekly Tuitition          $130.00
